Thrombocytopenia: High-Yield Board Summary
Severity classification, pseudo-thrombocytopenia pitfall, bleeding risk by count, workup, ITP and HIT management, and the exact platelet thresholds for procedures, transfusion, and activity.
Classification
Causes & Pseudo-Thrombocytopenia
Pseudo-Thrombocytopenia
Occurs when platelet clumping in the collection tube leads to a false low platelet count.
Any low platelet count should first be confirmed by repeating the test in a tube containing heparin or sodium citrate (instead of the standard EDTA tube).
Board trap: a single low platelet count on a routine CBC should never be acted upon (workup, hospitalization, treatment) without first ruling out pseudo-thrombocytopenia via a repeat test in a citrate or heparin tube.
Age-based screening pearl: any patient older than 60 years presenting with isolated thrombocytopenia should be evaluated for myelodysplastic syndrome and lymphoproliferative disorders.
Bleeding Risk by Platelet Count

Exam pearl: memorize these exact cutoffs — >50 asymptomatic, 20–50 bruising/petechiae with minor trauma, <10 spontaneous bleeding — as isolated numeric facts frequently tested directly.
Laboratory Evaluation
For All Patients
Peripheral smear — schistocytes indicate microangiopathic hemolytic anemia (TTP, HUS).
Additional Tests — Guided by Clinical Picture
- DIC panel: fibrinogen, D-dimer, coagulation factors
- Hemolytic panel: direct Coombs test, haptoglobin, LDH, reticulocytes
- LFTs, U/E (electrolytes)
- Viral serologies: HIV, H. pylori, hepatitis
- Stool antigen, fecal occult blood
- Vitamin B12 and folate
- TSH
- Antiplatelet antibodies
Board trap: schistocytes on smear should immediately trigger consideration of TTP/HUS — a hematologic emergency requiring urgent recognition, distinct from ITP (which does not show schistocytes).
Indications for Hospitalization
- Platelet count <10 × 10³/µL
- Evidence of hemolysis
- New neurologic or renal dysfunction
- Recent exposure to heparin products, associated coagulation abnormalities, or vascular findings (e.g., extremity pain, swelling, skin necrosis)
- Life-threatening hemorrhage (e.g., intracranial, GI, genitourinary)
- Anemia
- Leukopenia
- Leukocytosis
- Severe illnesses (e.g., sepsis)
Drug-Induced Thrombocytopenia
- Usually causes thrombocytopenia within 3–10 days of exposure
- Usually improves within 10 days of stopping the offending medication
Exam pearl: the onset (3–10 days) and recovery (within 10 days of discontinuation) windows are both testable — a new medication started roughly a week before thrombocytopenia onset should raise this on the differential.
Immune Thrombocytopenia (ITP)
- Definition: premature splenic destruction of antibody-coated platelets, with other hematopoietic cell lines remaining normal
- Causes platelet count <100 × 10³/µL
- Diagnosis of exclusion
- Often requires hospitalization
Treatment
- First-line: steroids, IVIG
- Recurrent or unresponsive cases: thrombopoietin receptor agonists or immunomodulators (e.g., rituximab)
- Splenectomy if ITP persists >1 year with no improvement on other medications
Board trap: ITP is isolated to the platelet line — if anemia or leukopenia is also present, reconsider the diagnosis (e.g., Evans syndrome, marrow infiltrative process, MDS) rather than assuming ITP.
Heparin-Induced Thrombocytopenia (HIT)
Treatment: discontinue heparin immediately when HIT is suspected, and start immediate anticoagulation with a non-heparin anticoagulant to prevent thromboembolic complications.
Board trap: despite the low platelet count, HIT is fundamentally a prothrombotic (not primarily bleeding) disorder — stopping heparin alone is insufficient; a non-heparin anticoagulant must be started to prevent thrombosis, not withheld out of bleeding concern.

Exam pearl: the higher the bleeding consequence of a complication (neurosurgery, eye surgery — where even small bleeds are catastrophic), the higher the required platelet threshold. Memorize the three tiers: 20 → 50 → 100.

Indications
- Platelet count <10 × 10³/µL
- Pre-procedure (per thresholds above)
- Active hemorrhage
Exam pearl: 1 platelet unit increases the platelet count by approximately 30–40 × 10³/µL — a favorite dosing/math calculation fact.
Activity Participation by Platelet Count

Board trap: activity restriction scales inversely with bleeding risk in graded tiers (10–20 / 20–50 / ≥50) — don't collapse this into a single blanket "avoid exercise" answer for any thrombocytopenic patient.
